How to refer

Women can self refer
|
To make a clinic appointment, a GP referral can be faxed to the Clinic Access Centre or women can phone the Access Centre directly.
|

Hours and days of service operation
|
Tuesday, Wednesday and Thursday - 9am to 11.30am.
|
Please bring to the appointment:
|
- Test results/Imaging
- Medicare Card
- Health Care Card
- Date of last pap smear test
- Blood group card
- List of medications.
|
- The hospital will already have the referral letter if you've been referred by your GP.
|
