Primary Care & Population Health Advisory Comittee

The Royal Women's Hospital Primary Care and Population Health Advisory Committee is appointed by and reports to the hospital's Board of directors.



Objectives


The committee will:
  • Advise the Board to ensure that there are processes and accountability for enhancing integration between the Women’s and the primary care service system and supporting health service engagement in population health initiatives; and
  • Identify population health issues and needs for both the local community and statewide, and consider these in service planning and delivery;
  • Enhance the provision of primary and secondary health services by The Women’s via coordination activities with other community health services, particularly primary care partnerships;
  • Prepare and ensure an annual primary care and population health work plan. This will involve developing the strategic directions for primary care and population health activities and prioritising and advising on resourcing needs.

Terms of reference


In consultation with key stakeholders, the Committee will:
  • Identify key health issues for population groups
  • Ensure the above issues inform The Women's annual strategic plans;
  • Identify the role of The Women's in these issues
  • Advise The Women's executive committee of Primary Care & Population Health Advisory Committee priority issues;
  • Identify support services/partnerships/activities required to fulfil The Women's role;
  • Assist The Women's to monitor resource allocation in relation to population health issues;
  • Access the broadest range of primary care and population health expertise to assist the committee in fulfilling its purpose;
  • Advise the Board in relation to primary care and population health issues; and
  • Recommend to the Board service developments and changes, and relationships for service delivery to:
a) Metropolitan populations;
b) Regional populations;
c) Rural populations;
d) Disadvantaged or special risk groups; and

  • Evaluate the committee annually.

Reporting


The committee reports quarterly to the Royal Women’s Hospital Board of directors.
