Introduction to Health Hub
Health Hub gives you greater access to your health information, strengthening the partnership between you and your healthcare provider.
There are 3 ways to sign up to Health Hub:
1. Health Hub online sign-up form
You can sign up to Health Hub through the website Patient Portal - Login Page.
You will need your mobile phone and current Medicare card details so we can verify who you are.
TIP – Download the Health Hub app to make things even easier. You can turn on notifications to remind you about your next appointment.

2. Health Hub sign-up text message
If you have an appointment booked at the Women’s, you may receive a text message with a secure link to sign up to Health Hub.
If you are worried about clicking a link, you can sign up using the online form or in person.
3. In person
Ask any of our friendly staff about signing up to Health Hub. You can also sign up at the Welcome Centre on the ground floor .
Welcome Centre opening hours: Monday to Friday: 8 am – 5 pm
Our staff can verify your identity and sign you up as a patient. They can also share access to your Health Hub by signing up a ‘proxy’ (parent, legal guardian, or medical decision maker). You must have a copy of your photo ID with you.
For your safety, signing up a proxy is only available in person at the hospital. Please see ‘Sharing access to your Health Hub’ for more information.
TIP – Download the Health Hub app to make things even easier. You can then turn on Face ID/ biometric login/passcode in your account settings.
Appointments: You can view past and future appointments in Health Hub.

Reviews: Find information about your recent outpatient clinic appointment. This can include clinic notes and referral letters.
Medical certificates: You can view medical certificates in Health Hub.
Test results: Some blood test and imaging results will be visible in Health Hub (usually within 4 days).
Health summary: Your Health Hub profile also includes a summary of your medicines, health conditions, and allergies. This means that you and your care team can find all important information in one place.
Questionnaires: For some medical and surgical clinics , a questionnaire will be sent to you through Health Hub. Once it’s available, you can complete it online at a time that suits you.
Parkville precinct: If you are receiving care at Peter MacCallum Cancer Centre, the Royal Melbourne Hospital or the Royal Children’s Hospital – you’ll be able to see those details too.
Pregnancy care companion: This provides useful reminders about upcoming appointments. It also includes helpful information about each stage of your pregnancy.
You can update your Health Hub profile to help your care team understand your needs.
Personal information
You can choose to share information such as your gender identity and sexual orientation. Sharing this information helps us provide respectful and inclusive care.
Disability questionnaire
You can complete a short questionnaire to let staff know about any visible or hidden disabilities. This helps us understand your needs so we can support you in the way that works best for you.
Providing this information helps your care team better understand your needs — without you needing to repeat it at every appointment.

If you’re concerned about your safety or privacy when using your device, or that someone may be monitoring your phone, you can speak with us in person.
Please come to the Welcome Centre and talk with a staff member about Health Hub before signing up.

You can set up Health Hub so that someone else can also access your account. We call this person a “proxy”.
They might be a parent, legal guardian or medical decision maker. It is important that it is someone who you trust and want to be part of your healthcare journey.
You can also sign up for proxy access in person at the hospital. Please speak to a staff member or the Welcome Centre on the ground floor to talk more about this.
If you decide you no longer want to share your Health Hub profile, you can remove proxy access at any time. Please speak to a staff member or the Welcome Centre.
You can also choose to share a one time link with your clinician to give them temporary access to your Health Hub account.
